What is a cerclage pessary?

The problem of miscarriage and threatened preterm birth is one of the most pressing challenges in modern obstetrics. The threat of miscarriage and preterm birth is a very common problem in patients with a complicated gynaecological and obstetric history (miscarriage in the previous pregnancy or infertility) and in patients with a multiple pregnancy. Today, we have some means of prevention at our disposal. One way to prevent miscarriage and preterm birth is to use a cerclage pessary. This is a small silicone medical device that is inserted into the vagina to hold the uterus in a certain position. The cerclage pessary may be inserted in the first or second trimester for patients with cervical shortening, after cervical conization performed before pregnancy and for prevention of cervical insufficiency (a history of pregnancy loss after week 12). Insertion of a cerclage pessary is a non-invasive alternative to the cervical cerclage procedure (a surgical procedure in which sutures are used).

The clinic iVF Riga offers insertion of a cerclage pessary. To select the size of a cerclage pessary, it is necessary to perform a vaginal ultrasound and estimate the number of foetuses. Insertion of a pessary is an outpatient procedure which does not require anaesthesia or hospitalization.
